Job Description:
A company is looking for a Care Manager Transition of Care.
Key Responsibilities
Evaluate member needs through post-discharge assessments and
medication reconciliation
Develop care/service plans and collaborate with healthcare teams
for member transitions
Facilitate education and resources for members and coordinate
services with providers
Required Qualifications, Training, and Education
Master's degree in Behavioral Health or Social Work, or a Degree
from an Accredited School of Nursing
2 - 4 years of related experience
Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN required
Must reside in Kentucky or Mississippi
For RN, must have a compact license or be willing to obtain one
