Job Summary:The Certified Diabetes Educator (CDE) is an RN
Clinical Program Coordinator and is responsible for the planning
and coordination of all aspects of care for the patients throughout
the continuum of services-from initial referral to follow-up care.
Serves as the primary coordinator for the interdisciplinary team,
including: physicians, nurses, social workers, dietitians,
pharmacists, financial analysts, case managers, and support staff.
Develops, coordinates, implements, and evaluates educational
programs/projects for patients, personnel, and the community.
Collects/documents data to meet clinical, financial, and regulatory
requirements. Participates in the development of guidelines,
protocols, pathways, and other performance improvement activities.
Serves as a patient advocate throughout the patient care process.
Represents the Medical Center at regional and national
meetings/conferences. The Clinical Program Coordinator provides
nursing care consistent with Nursing Standards of Care/Practice,
the Plan for Provision of Nursing Care, and the Plan for Provision
of Care to Patients, CSMC Code of Conduct, the Performance
Improvement Plan, the Patient Education Plan and Nursing Strategic
Plan. He/she maintains competency skills including the ability to
provide care to special patient populations and patient with
diverse cultural backgrounds. He/she can identify the ability of
the patients to follow directions/instructions and give consent.
He/she can identify safety concerns and institute measures to
provide a safe environment; and provide appropriate assessment,
intervention and evaluation of patients with consideration of
developmental milestones in the patient population. The
CDE/Clinical Program Coordinator is responsible for evaluation and
control of diabetes and other health risks to meet the goal of
diabetes education to improve the outcome before and during
pregnancy for women who have diabetes. Diabetes Educators are
certified by the National Certification Board for Diabetes
Educators. The CDE helps patients deal with the challenge of
diabetes before and during pregnancy and provides ongoing
Educational Requirements:BSN Required, MSN preferredBLS
requiredLicenses:California RN LicenseCertification as Diabetes
Education from the National Certification Board for Diabetes
Educators is required.Experience:Two years experience as an RN
required. Must have experience in diabetes education. Prefer
experience with diabetes in pregnancy in a high risk obstetric
clinical setting.Physical Demands: Carrying, hearing/listening,
lifting (up to 25lbs), Manual Dexterity, Pulling/Pushing, Seeing,
Sitting, Speaking, Squatting, Kneeling, Standing and Walking
