Registered Nurse (RN) | Case Manager Location: Mission Viejo, CA Agency: Cynet Health Pay: $3,019 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Mission Viejo, California, 92691! Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 2 years of acute medical care management experience performing care coordination. License: California state license required; pending license accepted but must be licensed upon start. Certifications: Required certifications in relevant areas. Must-Have: - Experience in assessing patients, care coordination, and discharge planning. - Ability to organize and participate in family meetings. - Knowledge of HMOs, IPAs, and relevant treatment options for patients with drug/alcohol abuse issues, mental health history, and those experiencing homelessness. - Competence in arranging durable medical equipment, skilled nursing facilities, and ambulance transport. - Proficiency in computerized documentation, specifically with the Epic charting system. Description: Job description includes performance of care coordination. Floating to different facilities may be required. Patient ratios are maintained at 1:25. Travel experience is mandatory. Weekend rotations will be part of the schedule. Commitment to working three of the five major holidays is required. References from two supervisory roles covering the last three years must be provided. All staff are expected to float among various facilities within the state. Refusal to float may result in termination of assignment.
