What You’ll Do Conduct comprehensive initial evaluations and assessments and promptly report findings to the physician/provider and Clinical Manager Develop, implement, and update individualized plans of care based on patient needs and physician orders Perform skilled physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests in the home setting Complete initial evaluations, treatment documentation, and re-evaluations every thirty (30) days; update plans of care as needed Complete comprehensive assessments and OASIS documentation accurately and on a timely basis, as applicable Observe, document, and report patient response, progress, and any significant changes to the physician/provider and care team Communicate and collaborate with physicians, providers, and interdisciplinary team members to ensure coordinated, high-quality care Provide education and support to patients and their families to promote safety, independence, and long-term success Assist with orientation, supervision, and consultation for Physical Therapy Assistants, as assigned You'll Love This Role If You Are Compassionate, dependable, and patient-focused Comfortable working independently in the home setting A collaborative team player who values communication and support Motivated by meaningful, one-on-one patient care Qualifications Master’s degree in Physical Therapy Active Physical Therapy license in good standing in the state of California At least one year of experience working as a Physical Therapist Must have a vehicle, a valid current driver’s license, and automobile insurance Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner Home Health and Home Care Home Base documentation experience Schedule Options We are currently hiring for Per Diem Physical Therapy opportunities.
