Job Description:
Position Purpose: The Patient Navigator role is designed to
provide comprehensive support to clients by identifying their needs
and guiding them through our appropriate administrative systems and
patient support services. This position involves warmly greeting
patients and extending assistance in waiting room areas, ensuring a
seamless and positive experience. Additionally, the Navigator will
actively address any questions or concerns patients may have,
contributing to an overall atmosphere of comfort and efficient
service delivery. Principal Responsibilities: Welcome all clients,
visitors, and individuals from the general public with a friendly
and professional demeanor. Engage in meaningful conversations with
clients and visitors to identify their informational needs. Provide
assistance to clients of diverse cultural backgrounds, aiding their
understanding of the Health System in terms of service access and
limitations. Establish and maintain effective communication
channels with clients and the healthcare team to ensure a smooth
and coordinated experience. Serve as a vital liaison between
clients and the medical staff, facilitating seamless interaction
and information exchange. Coordinate the dissemination of updates
and changes from front/back office staff to ensure accurate and
timely information flow. Assist clients in completing necessary
forms, streamlining administrative processes for enhanced
efficiency. Offer comprehensive information on on-site services and
promote health awareness to clients. Educate clients about
available benefits, including health coverage programs, and refer
them to Enrollment Specialists for eligibility screening. Act as a
representative of the clinic in matters related to the delivery of
healthcare services, working closely with the Clinic Administrator.
Collaborate with the outreach team to greet and receive new
clients, actively participating in the intake process. Engage in
community outreach initiatives to broaden the clinic's impact and
accessibility. Contribute to the maintenance of incentive program
logs, ensuring accurate and up-to-date records. Facilitate the
transportation of patients from the local hospital Emergency
Department to the E. Hollywood clinic using public transportation.
Attend general staff meetings to stay informed and actively
participate in the clinic's collective efforts. Adhere to all
organizational safety policies, procedures, and regulatory
requirements to ensure a safe environment for patients, staff, and
visitors. Participate in required safety and compliance trainings
and apply learned practices in daily work activities. Identify,
report, and help mitigate unsafe conditions, incidents, or hazards
in the workplace. Use equipment, tools, and supplies safely and
responsibly in accordance with organizational and regulatory
standards. Maintain awareness of infection control, occupational
health, and emergency preparedness procedures appropriate to the
role. Perform other duties as assigned by supervisor. Requirements:
Experience Required: Minimum 1 year of front desk or receptionist
experience. Demonstrated experience in effectively engaging with
clients and collaborating with staff from diverse socio-economic,
ethnic, and cultural backgrounds. Exceptional communication skills,
both verbal and written, to facilitate clear and concise
interactions. Bilingual proficiency in English and Spanish is
highly desirable. Flexibility and capability to travel between
different sites as required by the role. Possess a valid CA
Driver’s License, maintain a clean driving record, and be able to
travel to different sites. Capacity to collaborate effectively with
healthcare teams and contribute to a cooperative and
patient-focused care environment. At least 1 to 2 years of
experience in client support or community service roles.
Willingness to work some evenings and weekends, reflecting a
commitment to meeting the varied needs of our community and
