We are looking for CARING and COMPASSIONATE individuals to become a part of our Home Instead Pasadena family and join our mission of enhancing the lives of aging adults throughout our community.Job Description:The Home Care Case Manager position (i.e., Client Care Coordinator) performs a variety of duties that relate to client care including, but not limited to:

  • Care consultations with potential clients and family members
  • Utilizing consultative sales approach to determine each individual client's needs to provide solutions and create a tailored non-medical care plan
  • Client/CAREGiver introductions and field support
  • Community outreach and quality assurance
  • Continuously evaluating the care plan through ongoing communications and visits with clients and their families to ensure high-quality care, client satisfaction and retention as well as opportunities to increase service hoursPrimary Responsibilities:
    • Meet with potential clients and family members to discuss their needs and provide solutions in the form of a care plan
    • Conduct Service Inquiries and Care Consultations as needed following the consultative sales process
    • Maintain ongoing communication and consistent follow-ups with potential clients and families who have yet to commit to service
    • Work with other team members to coordinate various aspects of a client's care
    • On-Call Supervisor responsibilities at least one night per week
    • Conduct client/CAREGiver introductions
    • Create and maintain client and responsible party records documenting all quality assurance meetings
    • Plan and execute routine quality assurance visit
    • Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through the quality assurance program
    • Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers
    • Work with clients and their families on the various issues that may arise to ensure they are getting resolution to issue as well as receiving the services needed
    • Support community outreach and marketing initiatives to increase brand awareness
    • Manage Lead CAREGiver activities to support the quality assurance program
    • Conduct training classes on relevant senior topics with clients & CAREGiversQualifications:
      • Must have a Masters degree in Gerontology, MSW or a nursing degree/license (LVN or higher)
      • Required to have a minimum of 2 years of case management experience in client care in a medical or non-medical setting.
      • Must have the ability to present a professional appearance and demeanor.
      • Must be patient and congenial.
      • Must have computer skills and be proficient in Microsoft Office Suite.
      • Must be fluent in English, able to communicate information efficiently, possess strong work ethic, multi-task, able to follow through and complete assigned tasks.
      • Must be a good listener, detail oriented, and the ability to prioritize tasks effectively.
      • Applicant should be flexible and be available to work weekends and evenings as needed.
      • Must have a valid driver's license, reliable car and car insurance.
