Job Description Summary Responsible for assisting the office with managing all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. Tracking all referrals, admissions, and non-admits, including rational. Essential Job Functions/Responsibilities 1. Updates the office leadership with daily patient referral and intake operations, and assists with oversight of the establishment and implementation of intake policies and admission process. 2. Assists office leadership with maintaining compliance with all state, federal, and ACHC referral/intake regulatory requirements. 3. Assists with the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy in a timely manner. 4. Establishes and maintains positive working relationships with current and potential referral sources; providing updates when appropriate regarding the admission process. 5. Ensures seamless transition of patients to hospice care by providing direct oversight of patient education and preparation for hospice care, plan of care initiation, and coordination of care with multiple service providers. 6. Ensures insurance verification and authorization processes are completed timely and accurately. 7. Assists the office leadership in the preparation of QAPI, IDG, and other office meetings. 8. Assists with recruiting and orientation of the intake team, when appropriate. 9. Monitors community and customer perceptions through referral sources of Urgent Help Hospice, Inc. as a high quality provider of services. 10. Gathers, collates, and reports referral statistics including key customer referral trends to the office leadership. 11. Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs and reporting to the office leadership. 12. Maintains comprehensive working knowledge of Urgent Help Hospice, Inc. contractual relationships and ensures that patients are admitted according to contract provisions. 13. Participates in quality assessment performance improvement teams and activities. 14. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Urgent Help Hospice, Inc. 15. Initially, serves as field LVN in addition to coordinator responsibilities. Once growth indicates need for additional LVNs, this role will serve in the field only when absolutely necessary. 16. Additional responsibilities and tasks as directed by office leadership. The incumbents may be requested to perform job-related tasks other than those stated in this description. Position Qualifications 1. Ability to work independently and in a team environment. 2. LVN/LPN/CNA with current license to practice in California. 3. At least two (2) years experience in hospice care. 4. Ability to market aggressively and deal tactfully with customers, patients, families, and the community. 6. Working knowledge of medicare, medi-cal, and other payor sources (or willingness to learn). 7. Demonstrates good communications, negotiation, and public relations skills. 8.
